This form is the first step to request speaker or instructor representing CEBA for your event. Please take your time to thoughtfully complete this questionnaire so that we can meet your needs as swiftly as possible.
Preference will be given to CEBA member organizations and the associated community. Please provide at least six weeks advance notice of your event.
